ALL PATIENTS will be asked to present their insurance card starting January 1, 2024.
If you have not yet received an updated card from your insurance company, but have been sent updated insurance ID and/or group information, please provide that information.
If you do not have an updated card, you will be asked to present your current insurance card again.
If updated insurance information is not provided at the time of service, any remaining balance you owe PT@acac may be the responsibility of you, the patient.
