ATTENTION PATIENTS:

As of January 1, 2026, all patients are required to provide a copy of their insurance card at time of visit to our clinics.

Please notify the office team at your clinic if your insurance has changed.

Failure to provide insurance information at time of visit may result in a patient balance for that visit.

• If we do not have current/valid insurance information, you will be billed at our self-pay rate of $100 per visit.

• If there is a delay in receiving your information and authorization cannot be obtained for your visit, you will be billed at our self-pay rate of $100 per visit.

• Updated information can be provided to our office anytime between now and December 31.
